All patients that are referred to Nephrology Physicians, LLC, by their primary care physician will receive:

A telephone call will be made from our office to let you know that you have been referred to us.

During the telephone notification call the New Patient Coordinator will ask you 7 questions from our "New Patient Checklist". Download Our New Patient Checklist Here.

We will fax a list of request to your referring physician's office for your medical records and for them to order specific blood tests.

We will mail a packet of information that will need to be completed in full and brought with you to your first appointment.

When we have an appointment opening for you and have received all of your medical records and required blood test we will contact you via telephone to set up the appointment. If you are able to make this appointment we will follow up will a letter stating your appointment date, time and the physician you will be seeing.

NOTE: Please plan to spend approximately 1.5 hours in our office for your first visit.

When you come in for your first appointment, you will need to BRING THE FOLLOWING with you:

1. ALL of your prescription bottles, including any over-the-counter medications.
2. Your insurance, Medicare and/or Medicaid cards.
3. IF your referring physician has given you copies of your medical records, please bring them with you.
4. The COMPLETED packets of information that was mailed to you containing: (Patient Information Packet (3 pages); Statement of Permit Payment; Medicare / Commercial Insurance Patients (1 Page); History and Review of Systems (5 Pages); Notice of Privacy Practices (9 Pages); Statement of Financial Policy (6 Pages) and Medicare / HCFA – Secondary Payor Questionnaire (3 Pages).
